In most cases the transmission cooler which is located inside the radiator on vehicles with an automatic transmission can rupture and mix oil with the coolant and usually looks like a strawberry milk shake.

bad water pump seal causes oil into the coolant, the seal separates the water from oil. once the seal is bad it will leak oil into the coolant side, specially when the weep hole in the pump is plugged. replace the pump to correct the problem.

Year, make ,model please. In most cases, its a head gasket leak.
busted water jacket in the head or blown head gasket between water jacket and cylinder. this will cause it to leak into the water line. and what goes into the cylinder comes out of the exhaust in the form of steam.
